Caseworker B - Intake

About the Role

The Caseworker B - Intake position requires independent judgment and a strong understanding of professional principles. The role involves assessing families and individuals to develop treatment plans and coordinating services with community agencies.

Responsibilities

• Performs case management activities involving complex and difficult problems.
• Conducts initial needs assessment and determines client’s/family’s history.
• Reviews case records and evaluates social information concerning families.
• Implements treatment plans and provides a variety of social services referrals.
• Participates in interdisciplinary team discussions to set treatment goals.
• Monitors and evaluates activities of contracted service agencies.

Requirements

• Bachelor’s degree in a relevant field.
• At least one year of related professional experience.
• Valid Colorado Driver’s License required.
• Successful completion of a criminal background check.
