Clinical Pharmacist

About the Role

As a Clinical Pharmacist, you will engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care. Your role will include supervising pharmacy technicians and identifying medication safety events to enhance patient outcomes.

Responsibilities

• Educate and counsel patients, families, and healthcare team members on safe and effective medication use.
• Provide safe, timely, and cost-effective medication therapy.
• Engage in direct patient care and interdisciplinary rounds.
• Supervise and mentor pharmacy technicians and students.
• Identify, report, and help resolve medication safety events.
• Participate in quality improvement initiatives.

Requirements

• Bachelor's degree in Pharmacy required.
• Licensed Pharmacist (RPH).
• 1+ year of hospital experience preferred.
• Strong organizational skills.
• Ability to follow verbal and written instructions.
• Proficiency with EHRs and pharmacy informatics systems.
