Licensed Practical Nurse (LPN) Skilled Nursing - 7pm-11pm Shift

About the Role

Join Wind Crest as a Licensed Practical Nurse providing clinical care to residents in the skilled nursing neighborhood according to nursing standards of practice. You will develop relationships with residents and families using a consistent caregiver approach, assess and document clinical conditions, and supervise care associates. The schedule is a 7pm-11pm rotating shift, with a week of daytime orientation, plus a $1/hour night shift differential.

Responsibilities

• Develop relationships with residents and their families using a consistent caregiver approach
• Assess, observe, and evaluate residents' clinical condition and document findings in the electronic medical record
• Update or modify care plans as needed to reflect changes in resident care and service needs
• Supervise and lead care associates
• Administer medications timely, accurately, and according to orders
• Participate in clinical indicator reviews including falls, wounds, nutrition, and antipsychotic use

Requirements

• Licensed by Colorado as a Licensed Practical Nurse
• CPR certification required
• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred
• Experience with electronic medical records is preferred
• A passion to care for seniors and be a strong resident advocate
