About the Role
The position involves providing investigation, assessment, case management, and service coordination for at-risk adults in Douglas County. The caseworker will develop and implement case plans to address the needs of these individuals, ensuring their safety and well-being.
Responsibilities
- Provide investigation, assessment, case management, and service coordination for disabled and elderly adults at risk of abuse, neglect, and exploitation.
- Develop and implement case plans addressing various client needs while promoting independence.
- Handle crisis intervention, family mediation, and service/resource coordination, ensuring follow-ups are conducted.
- Screen reports on the APS intake hotline, providing appropriate referrals for reports not meeting APS criteria.
- Maintain knowledge of community resources and provide them to clients and customers.
- Educate the community on adult protection issues and provide support to agencies dedicated to serving at-risk adults.
Requirements
- Bachelor’s degree in human behavioral sciences or health care related courses.
- One year of professional casework experience after obtaining a degree, or a Master’s degree in social work.
- Possess a valid Colorado Driver’s License and comply with background check requirements.
