About the Role
Ensures clear and effective communication among healthcare team members. Provides timely updates to patients and families about their care and discharge plans. Coordinates with various departments to facilitate smooth transitions and avoid delays. Develops and implements efficient discharge planning protocols. Assesses patients’ readiness for discharge and ensures all necessary paperwork and instructions are completed. Arranges follow-up appointments and provides patients with information on post-discharge care. Coordinates patient transfers within the facility and to external locations. Ensures all relevant medical information is accurately communicated during transfers. Monitors and addresses any issues that arise during the transfer process to ensure patient safety and continuity of care. Assesses patients’ physical, psychosocial, cultural, and spiritual needs through observation, interviews, medical records, and clinical data. Provides therapeutic interventions, including IVs, wound care, medication administration, catheterization, and specimen collections. Educates patients and families by assessing learning readiness, needs, and ability, and evaluating the effectiveness of teaching.
