Licensed Practical Nurse (LPN) Skilled Nursing - 7pm-11pm Shift

About the Role

The LPN/LVN provides clinical care to residents according to nursing standards, maintaining a consistent caregiver approach. This role includes assessing residents' clinical conditions, updating care plans, and supervising care associates.

Responsibilities

• Develop relationships with residents and families
• Assess and evaluate residents' clinical conditions
• Update care plans as needed
• Supervise care associates
• Administer medications timely and accurately
• Participate in clinical outcome meetings

Requirements

• Minimum of 1 year experience in assisted living or long-term care
• Licensed by Colorado as a Licensed Practical Nurse
• CPR certification required
• Passion to care for seniors
• Experience with electronic medical records preferred
