About the Role

In this role, you will assess patients' physical, psychosocial, cultural, and spiritual needs while collaborating with an interdisciplinary team. You'll develop and implement patient care plans, ensuring that each patient's health, safety, and rights are advocated for.

Responsibilities

• Assess patients’ needs and develop care plans.
• Implement therapeutic and diagnostic interventions.
• Advocate for and protect the health of patients.
• Provide patient and family education.
• Coordinate group visits for high-risk populations.
• Participate in patient experience improvement initiatives.

Requirements

• Registered Nurse (RN) license required.
• Basic Life Support – CPR certification required.
• Associate’s degree in Nursing required.
• 1+ years of medical office experience preferred.
• Excellent communication and documentation skills.
