About the Role

This role involves assessing patients’ needs and developing care plans in collaboration with a medical team. You will engage with patients to promote their health and recovery through various nursing interventions.

Responsibilities

• Assess patients' physical, psychosocial, cultural, and spiritual needs
• Develop and implement patient care plans
• Advocate for and protect the health, safety, and rights of patients
• Provide patient and family education based on assessment
• Encourage patient engagement in health care
• Coordinate and implement group visits for high-risk patients

Requirements

• Registered Nurse (RN) license
• Basic Life Support – CPR Certification
• 1+ years of medical office experience (preferred)
• Associate’s of Nursing (required)
• Bachelor’s of Nursing (preferred)
• Experience with Electronic Health Records (preferred)
