Care Manager RN

About the Role

This position involves assessing, planning, implementing, coordinating, monitoring, and evaluating care options and services needed to support individual health needs. The Care Manager RN advocates for clients, educates them on treatment options, and facilitates communication within the healthcare team.

Responsibilities

• Demonstrates critical thinking skills when utilizing the nursing process to meet client’s health care needs.
• Establishes collaborative partnerships with clients to examine health care needs and decisions.
• Advocates and educates clients and families about treatment options and community resources.
• Facilitates communication between members of the health care delivery team to minimize fragmentation in services.

Requirements

• Graduate of an accredited nursing program.
• State licensure as a Registered Nurse (RN).
• 2 years of nursing experience.
