Case Management Manager

About the Role

The Case Management Manager is responsible for coordinating services through a clinical, financial, and psychosocial approach to the continuum of care. This role involves conducting clinical reviews and collaborating with healthcare teams for effective discharge planning.

Responsibilities

• Coordinate services through an interdisciplinary process.
• Perform concurrent clinical reviews to determine the appropriateness of admissions.
• Handle telephonic clinical reviews with claims insurance personnel.
• Facilitate communication between patients, families, and healthcare teams.
• Identify quality and/or risk issues and report them appropriately.
• Support discharge planning and assess patient needs.

Requirements

• Hold LCSW or RN license.
• Experience in case management or healthcare management.
• Strong communication and collaboration skills.
• Ability to work in a fast-paced healthcare environment.
