Responsibilities

• Performs and documents initial and ongoing assessments of patient's condition.
• Establishes a plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires.
• Provides education to the patient and family/caregiver about patient deficits and information regarding appropriate selection/use of adaptive equipment and support programs. Plans and/or assists with patient discharge from therapy services and treatment setting.
• Performs all underlying duties of the job including but not limited to supervising support personnel according to established licensing laws, state regulations and practice standards.
• Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action. Requirements:
• * Licensed Physical Therapist
• Bachelor's degree in Physical Therapy required. Preferred: Master's or Doctorate degree in Physical Therapy. Required Licensure/Certification: State licensure as a Physical Therapist. Minimum Experience: None required.
• BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire. Compensation

• Base pay: $41.43 - $62.15/hr
